Know Your Forms or KYF for short!

Many times a client may not know what type of Notary Certificate they need since it may not be included. The good news is, a Notary Public will have copies of the proper certificate. They are known as the Acknowledgement and the Jurat. The Acknowledgment is simply someone Acknowledging their signature on a document to be notarized and it will have more wording on this particular document. See below:


Who proved to me on the basis of satisfactory evidence to be the person(s) whose name(s) is/are subscribed to the within instrument and acknowledged to me that he/she/they executed the same in his/her/their authorized capacity(ies)...


The Jurat requires you to give an Oath or Affirmation, swearing to the truthfulness of the information in the documents. The Jurat will have less language written and will look like the following sample below:


Subscribed and sworn to (or affirmed) before me on this _____

day of ____________, 20__, by________________________,

proved to me on the basis of satisfactory evidence to be the

person(s) who appeared before me.....


If you are not sure if you have the proper notary documents or how to complete the forms provided to you, you should always defer back to your Attorney, Benefits Administrator, State Agency or who ever is requiring you to have the documents notarized in order to fulfill your request.
